The application process with MedMax RX is extremely simple. Download our customer onboarding form  by clicking the link below. If you are planning on purchasing controlled medications, click and download the controlled medications form and the customer onboarding form. For all controls customers a copy of your 90 day DUR will need to be submitted along with your application.
